The legendary Secret Cow Level has made its return in Diablo 4's Lord of Hatred expansion, but getting there is no simple task. Players have already discovered that the portal is almost accessible, though it requires a long chain of preparation before you can actually step through.
To start, you need to have completed the main campaign for the expansion. After that, the hunt begins for a series of obscure items and triggers scattered across the new zones. The first step involves collecting three specific ingredients: a Worn Idol, a Rusted Bell, and a piece of Old Cheese. These are not guaranteed drops. You will need to farm specific elite enemies in the Tormented Highlands and the Verdant Valleys, with some reports suggesting a higher drop rate during the Helltide events.
Once you have all three items, you must travel to a hidden altar located beneath the Tree of Whispers. Interacting with the altar while holding the items in your inventory will cause a spectral cow to appear. Do not attack it. Follow the cow for about two minutes as it leads you through a series of underground passages. It will eventually stop at a sealed stone door. This is where the current community effort stalls.
Data miners have found that the door requires a fourth item, a "Moo-ved Key," which has not yet been discovered. Players believe it may be tied to a world boss that only spawns under specific conditions, or perhaps a secret quest chain involving the new Kurast Undercity. Until that final piece is found, the Cow Level remains locked. But the hunt is very much alive, and the community is closing in on the secret.
